Findings: Alleged Victim (AV) relies on facility staff to meet his/her daily needs and has impaired cognition. According to an investigation, on or about, September 9, 2025, the Alleged Perpetrator 2 (AP2) used his/her body to move AV out of a room then pushed AV down onto a couch, pointed a finger at AV and scolded AV. W1 was contacted twice during the shift regarding AP2's actions towards AV. W1 did not immediately remove AP2 from their duties. AP2's actions are considered neglect and constitutes abuse. The facility failed to provide a safe environment and assure resident's rights, which is considered neglect of care and constitutes abuse.
