Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV is a high fall risk with a history of falls. AV moved into the facility on September 9, 2025. According to an investigation, AV experienced a fall on or about September 9, 2025, resulting in AV hitting their head. On or about, September 11, 2025, AV slipped out of bed, resulting in two rib fractures. The facility failed to provide appropriate services, including care planning and/or implementation of interventions, which is a violation of resident rights, is considered neglect of care and constitutes abuse., which is a violation of resident rights, is neglect of care which constitutes abuse.
