Findings: The Alleged Victim (AV) relies on the facility to provide care and services. According to an investigation, AV suffered multiple falls and the facility did not implement additional interventions nor a change of condition for AV after the falls started occurring. The facility failed to do a change of condition and implement additional interventions for AV, which is a violation of resident rights, is neglect of care which constitutes abuse.
