Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV is a high fall risk with a history of falls. AV is care planned for hourly checks. According to an investigation, on or about September 11, 2025, AV experienced an unwitnessed fall, resulting in pain. Facility staff had not checked on AV for approximately 3 hours. The facility failed to provide appropriate services, relating to following the care plan, appropriately care planning, and/or implement interventions to mitigate the risk of injury, which is a violation of resident rights, is neglect of care which constitutes abuse.
