Findings: The Alleged Victim (AV) resides in a secured community and relies on the facility to ensure their safety. According to an investigation, on or about September 14, 2025, Alleged Perpetrator 2 (AP2) assisted the AV to exit the secured memory care community. The allegation that the Alleged Perpetrator 2 neglected the AV was investigated and determined to be inconclusive. There was no documented evidence AP2 had received training related to the specialized needs of a memory care community. The facility failed to provide the oversight and supervision necessary to maintain the safety of the AV by not ensuring the training of AP2, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
