Findings: According to the documentation, the Alleged Perpetrator 2 (AP2) failed to protect the Alleged Victim (AV) from verbal abuse. on or about September 16, 2025, AV reported to facility staff that AP2 got in their face and yelled at them after they called for assistance. It was confirmed by the facility staff that AP2 had been coached on their behavior and verbal abuse towards residents in the past after another verbal altercation. The AP2 failed to protect the AV from verbal abuse which caused emotional distress, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
