- The facility failed to provide a safe medication administration system to ensure the AV’s medication was administered as ordered. AV did not get his/her prescribed medication from September 11,25 to September 17, 2025. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
