Findings: Alleged Victim (AV) relies on the facility for his/her care needs. AV was out of the facility from July 1, 2025, to July 31/25. When AV returned to the facility AV noticed his/her wallet was missing, staff located the wallet then AV noticed $40 missing and cards appeared to have been re-arranged in the wallet. Facility staff report AV's door was locked every time staff needed to go into the apartment while AV was out of the facility. In September 2025 AV's family discovered unauthorized transactions on AV's account. Alleged Perpetrator 2 (AP2) was employed by the agency and covered shifts at the facility. AP2 admitted that he/she would write down residents’ credit and debit card numbers then use them. AP2 failed to protect AV from financial, which is neglect of care and constitutes abuse. The facility failed to protect AV from financial exploitation, which is a violation of Oregon Administrative Rules.
