Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV was care planned to not be left unattended in the courtyard. According to an investigation, on or about September 28, 2025, AV was unattended in the courtyard and experienced an unwitnessed fall, resulting in a skin injury. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care which constitutes abuse.
