Findings: Witness #1 (W1) is known to have aggressive behaviors towards other residents. On or about October 4, 2025, W1 had an altercations with another resident at approximately 3:00 PM and at approximately 3:30 pm, W1 engaged in an altercation with the Alleged Victim (AV). W1 pulled on AV's wheelchair and flicked AV on the cheek. W1 has multiple interventions in place, however, W1 has no known triggers, which makes interventions difficult. Staff are to redirect W1 from other residents to avoid altercations. The facility's failure to ensure oversight of W1 to ensure safety of others is a violation of resident rights, is considered neglect of care and constitutes abuse.
