Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations. Interventions included keeping W1 away from others when appearing agitated and to monitor W1 when near AV. On or about October 3, 2025, W1 approached AV in the dining room and pushed AV into a table. Staff were present at the time of the incident but did not intervene to keep the residents apart. Respondent failed to ensure staff intervened when W1 and AV were in proximity to avoid an altercation, placing AV at risk for harm. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
