Findings: Alleged Victim (AV) lives at Respondent’s facility and relies on facility staff for all care needs. AV uses a wheelchair for mobility, and is a two person assist with transfers. AV has a history of falls, including incidents in August 2025, and September 2025. The facility failed to develop sufficient interventions to mitigate risk to AV. On or about October 5, 2025, facility staff found AV on the floor in AV's apartment, AV had fallen and scooted towards the air conditioner. Respondent failed to develop and implement suitable interventions. This constitutes abuse by neglect.
