Findings: Alleged Victim (AV) lives at Respondent’s facility and relies on the facility staff to provide appropriate supervision and care. AV has a history of falls, including an incident that occurred on July 4, 2025. However, the facility failed to develop sufficient interventions to mitigate risk of falls to AV. On or about July 22, 2025, AV lost balance and fell, when attempting to reach for something resulting in AV hitting h/h head and complaining of back pain. EMS transported AV to hospital. Respondent failed to develop and implement suitable interventions, which constitutes abuse by neglect.
