Findings: Alleged Victim (AV) lives at Respondent’s facility and relies on the facility staff to provide appropriate supervision and care. AV has a history of falls, including an incident that occurred on July 22, 2025, that resulted in EMS transporting AV to the hospital for evaluation. However, the facility failed to develop sufficient interventions to mitigate risk of further falls to AV. On or about July 30, 2025, AV fell and was found in the same location on the floor as the prior fall on July 22, 2025. AV reports pain in back and head, EMS transported AV to the hospital for evaluation. Respondent failed to develop and implement suitable interventions, which constitutes abuse by neglect.
