Findings: Alleged Victim (AV) lives at Respondent’s facility and relies on the facility staff to provide appropriate supervision and care. AV has a history of falls, including two prior incidents that occurred in July 2025, that resulted in EMS transporting AV to the hospital for evaluation, and an incident on August 10, 2025. However, the facility failed to develop sufficient interventions to mitigate risk of further falls to AV. On or about August 15, 2025, AV was found on the floor of AV's living room on AV's back. AV reported hitting head, and pain at level 9 in back, neck and hip. AV was transported by EMS to the hospital for evaluation. Respondent failed to develop and implement suitable interventions, which constitutes abuse by neglect.
