Findings: On or about September 25, 2025, the Alleged Victim (AV) needed to be escorted to the hospital for lab work. Alleged Perpetrator #2 (AP2) accompanied AV to the hospital AV told AP2 he/she needed to use the restroom. AP2 was calling the transport company to come back to pick them up earlier and did not see AV exit the bathroom. AV left the hospital and boarded a bus to his/her old business. Staff found AV at his/her old business about 40 minutes later. An investigation determined that AP2 wasn't properly trained prior to escorting AV. The investigation determined no abuse by AP2. The facility's failure to ensure that AP2 was properly trained on escorting staff placed AV at risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
