Findings: Alleged Victim (AV) resides in the Respondent’s memory care facility and is dependent on facility staff for all care needs and safety. AV has cognitive decline and is oriented only to self. Facility staff failed to provide adequate supervision and a secured environment for residents, including AV, while conducting activities. On or about October 9, 2025, AV was participating in an arts and crafts activity when AV obtained a pair of scissors and cut AV’s left index finger. Respondent failed to maintain a safe environment and adequate oversight. This constitutes abuse by neglect.
