Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, Alleged Perpetrator 2 (AP2) failed to provide AV with necessary supervision which resulted in AV having an altercation with Witness 1 (W1) in the dining room. As a result of this altercation, AV was struck and received a skin tear. AP2 failed to provide AV with necessary care and services which is a violation of resident rights, is considered neglect which constitutes abuse. The facility failure is a violation of Oregon Administrative Rules.
