Findings: Witness #1 (W1) has a history of attacking other residents and staff are to keep eyes on W1 while in the common area. On or about October 20, 2025, the Alleged Victim (AV) and W1 hit each other after W1 wheeled down the hall and tried to take AV's coffee. W1 suffered a scratch to his/her face, and AV suffered a scratch on his/her nose. Staff did not think that W1 could self propel in his/her wheelchair and took eyes off W1. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
