Findings: The Alleged Victim (AV) has a history of attacking other residents and staff are to keep eyes on AV while in the common area. On or about October 20, 2025, Witness #1 (W1) and AV hit each other after AV wheeled down the hall and tried to take W1's coffee. AV suffered a scratch to his/her face, and W1 suffered a scratch on his/her nose. Staff did not think that AV could self propel in his/her wheelchair and took eyes off AV. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-01362 $375.00 fine assessed
