- Findings
- Witness 1 (W1) is very anxious and becomes agitated and aggressive quickly. On or about October 17, 2025, W1 got physical with Alleged Victim (AV) which resulted in AV getting his on his/her face. Respondent failed to care plan appropriately resulting in AV being physically assaulted by W1, which lead to ensure the safety of AV. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
