Findings: The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, on or about October 19, 2025, AV experienced a fall at approximately 2:39 am resulting in significant pain. The facility did not address AV’s pain until approximately 1:00 pm leaving AV in continued pain for several hours, causing AV to not feel well. The facility failed to provide appropriate services regarding pain management, which is a violation of resident rights, is neglect of care which constitutes abuse.
