Findings: The Alleged Victim (AV) receives his/her medication from facility staff. On or about October 24, 2025, through October 26, 2025, AV did not receive his/her scheduled narcotic medication. Facility staff did not re-order AV's medication timely, leaving AV without medication, suffering from increased pain and unreasonable discomfort. Respondent failed to ensure staff re-ordered medication timely, to avoid AV from missing his/her medication. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
