Findings: The Alleged Victim (AV) and Witness 1 (W1) rely on the facility for their care. AV and W1 have a history of behaviors and resident-to-resident altercations. According to an investigation, on or about, October 22, 2025, W1 pushed AV, resulting in AV pushing W1 back, shoving W1 into the wall, and holding W1’s neck. The facility failed to provide services, to include providing a safe environment, properly care planning and/or implementing interventions, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
