Findings: The Alleged Victim (AV) relies on the facility for their care. AV is care planned for two-person staff assistance, using a gait belt, for all transfers and requires one-person staff assistance with ambulation, using a wheelchair. AV has a history of falls and is known to self-transfer without asking for staff assistance. According to an investigation, on or about October 29, 2025, AV experienced a fall, resulting in knee pain. The facility failed to provide services, to include properly care planning and/or implementing interventions, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
