Findings: The Alleged Victim (AV) relies on the facility to provide a safe physical environment. According to an investigation, on October 24, 2025, the AV's flat screen television fell off the AV's dress and hit them in the head. The TV fell off the dresser again on October 25, 2026, landing on the AV. The facility failed to properly address a known safety hazard to provide a safe environment for the AV, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
