Findings: The Alleged Victim (AV) relies on the facility for their care. AV has a history of falls and is care planned for staff to provide physical assistance with transferring and all ambulation/mobility needs. According to an investigation, on or about October 11, 2025, while on a shopping trip with the facility, AV was ambulating without assistance and fell in a parking lot. AV received multiple injuries, including a bump on the right side of their head and scrapes on AV’s left knee and left elbow. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP26-00124 $250.00 fine assessed
