Findings: Alleged Victim (AV) and Witness 1 (W1) live at Respondent’s facility and relies on the facility staff to provide safety. W1 has a history of anxiety, agitation resident-to-resident, and staff altercations. AV and W1 have a romantic relationship, however, AV has expressed no longer feeling safe around W1. W1's interim service plan reflects W1 was put on a 1:1 for aggressive behavior. However, facility staff failed to follow W1 interim service plan for 1:1 placing AV at risk of serious harm. On or about October 10, 2025, Witness 3 (W3) came on shift and the 1:1 was nowhere to be found. AV's door was cracked open and W1 was in AV's room. Respondent failed to follow W1 service plan and interventions. This constitutes abuse by neglect.
