Findings: The Alleged Victim (AV) relies on the facility to provide care and services. The resident moved into the facility on or about 10/24/25 and was receiving wound care. As part of the care and services the facility should have been assisting AV with wound care. The AV also had a change in weight bearing status that was not noted by the facility in the service plan. The facility failed to provide care and services for AV, which is a violation of resident rights and is neglect which constitutes abuse.
Sanction: ALFCP26-00193 $450.00 fine assessed
