Findings: On or about November 7, 2025, Witness #1 (W1) approached the Alleged Victim (AV) while he/she was sitting at the table awaiting breakfast. W1 pulled AV's hair. W1 is care planned to be kept separated from other residents and be redirected to avoid altercations. Although staff were present, this altercation wasn't prevented. Respondent failed to ensure staff followed the service plan and ensure the safety of residents from altercations. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
