Findings: The Alleged Victim (AV) relies on the facility for their care and is a fall risk. According to an investigation, on or about November 11, 2025, AV experienced a fall in AV’s room, causing bruising and an abrasion to their left eye and forehead and bruising to AV’s right elbow. The facility failed to provide appropriate services, including care planning and/or implementation of interventions, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
