According to an investigation, on or about November 13, 2025, AV experienced another fall in AV’s room, posing a risk of harm. The facility failed to provide appropriate services, including care planning and/or implementation of interventions, which is a violation of resident rights, is considered neglect of care and constitutes abuse as defined in 411-020-0002(1)(b)(A)(ii).
