Findings: On or about November 9, 2025, Witness #1 (W1) hit the Alleged Victim (AV) in the face. W1 has a long history of hitting other residents, and staff are to keep W1 in their sight in common areas. On this date, no staff were watching W1 when he/she engaged in an altercation with AV. Neither AV nor W1 was injured, however, both residents were placed at risk of harm. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-01408 $250.00 fine assessed
