Findings: The Alleged Victim (AV) relies on the facility for their care. AV is care planned for staff to assist with putting on/taking off AV’s compression stockings daily and the compression socks to be washed and hung up to dry for the next day. According to an investigation, on or about November 12, 2025, AV was found to have a 7 cm in width, 5 cm in length and 0.2 cm deep stage 3 wound on AV's left heel. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
