Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment and follow the AV's care plan. On or about 09/24/2025, AV suffered a fall in their room when going to the bathroom. AV suffered a fracture and other lacerations, along with pain in other extremities. The facility failed to install a soft light in AV's room as indicated by the service plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
