Findings: The Alleged Victim (AV) relies on the facility for cares and to provide a safe environment. Witness 1(W1) and AV have had previous altercations at the facility, and the facility did not have any interventions in the service plan to try and mitigate the behaviors between W1 and AV. The facility's failure to implement any interventions to try and prevent behaviors and altercations between AV and W1 is a violation of resident rights, is considered neglect which constitutes abuse.
