- Findings
- The Alleged Victim (AV) relies on the facility to ensure their rights. According to an investigation, on or about November 20, 2026, Alleged Perpetrator 2 (AP2), physically held the AVs apartment door closed, preventing them from exiting. AP2 involuntarily secluded the AV. The AV experienced no negative outcome. The facility did not keep AV free from involuntarily seclusion, which is a violation of Oregon Administrative rules.
