Findings: The Alleged Victim (AV) relies on the facility for their care. According to an investigation, on or about November 23, 2025, AV experienced an unwitnessed fall in their apartment, resulting in an abrasion to their right finger and a scalp hematoma on the forehead that turned into a bruise. It was discovered that the motion sensors that AV’s PCP ordered were not added to the care plan and/or put into AV's tasks to be completed by care staff when they arrived in October 2025. The facility failed to appropriately care plan and/or implement interventions, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
