Findings: The Alleged Victim (AV) relies on the facility to ensure safety and resident rights. Witness 1 (W1) is a resident of the facility and has a history of entering other resident rooms. Interventions in place for W1 include re-direction away from other residents and staff are to ensure that W1 is not standing in front of, knocking on, or attempting to open the doors of other residents. According to an investigation, on or about December 1, 2025, W1 was not re-directed away from the AV's room. W1 entered the AV's room and struck the AV in the face resulting in bleeding, The AV was sent to the emergency department and diagnosed with a scleral hemorrhage of the left eye and an abrasion. The facility failed to follow the care plan for W1 and implement interventions, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
