Findings: The Alleged Victim (AV) attempts to exit the facility and has a history of elopement. According to an investigation, the AV eloped from the facility on October 21. 2025, November 25, 2025, and on December 2, 2025. The facility failed to provide appropriate services according to Alleged Victim’s needs, relating to care planning and appropriate interventions, to mitigate the risk of the AV eloping from the facility, placing them at risk of harm. This failure is a violation of resident rights, is neglect of care, and constitutes abuse.
