Findings: The Alleged Victim (AV) relies on the facility for their care. AV has a condition that causes tremors and AV's hands to flail, resulting in spilling and not being able to get fluids into their mouth from a cup. AV has a history of dehydration with a previous hospital admission. According to an investigation, on or about December 5, 2025, went to the emergency room for dehydration. According to witnesses, AV has been observed on multiple occasions to be sitting in the dining room with an empty cup and liquids spilled around AV. The facility failed to appropriately plan care for AV’s hydration needs, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
