Findings: The Alleged Victim (AV) relies on the facility for their care. Witness 1 (W1) had a history of inappropriate behaviors towards AV. According to an investigation, on or about November 30, 2025, W1 touched AV's arm, chest and inner thighs in the dining room, resulting in unreasonable discomfort and loss of personal dignity. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
