Findings: The Alleged Victim (AV) relies on the facility for their care. Witness 1 (W1) had a history of inappropriate behaviors towards AV. According to an investigation, between December 4, 2025 and December 7, 2025, W1 inappropriately touched AV multiple times, resulting in unreasonable discomfort and loss of personal dignity. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
