Findings: The Alleged Victim (AV) relies on the facility for their care, including medication management. According to an investigation, between November 30, 2025, and December 3, 2025, AV did not receive their pain patch medication as it could not be located, causing AV pain. It was discovered that facility staff stored the medication in the overflow cabinet. The facility failed to administer medication as ordered, which is a violation of resident rights, is neglect of care which constitutes abuse.
