Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment and provide services. On or about December 7, 2025, AV was able to leave the secured facility and facility staff were unable to locate AV. The local hospital called and reported that AV was brought there by a person that found AV in the community. The facility failed to provide a safe environment for AV, which is a violation of resident rights and is considered neglect which constitutes abuse.
Sanction: RCFCP25-01488 $375.00 fine assessed
