Findings: The Alleged Victim (AV) relies on the facility for their care, including medication administration. According to an investigation, on or about December 7, 2025, Alleged Perpetrator 2 (AP2) left AV’s medication on the dining room table at approximately 8:00am in AV’s room and left. Facility staff found AV’s medication on the table at approximately 7:26pm. AP2 failed to give medication as ordered and failed to observe AV take their medications, posing a risk of harm. AP2’s actions are a violation of resident rights, are considered neglect of care and constitutes abuse. The facility failed to ensure medication was administered as ordered, which is a violation of Oregon Administrative Rules.
