Findings: The Alleged Victim (AV) relies on the facility for their care. Witness 1 (W1) is known to have behaviors. On or about December 7, 2025, W1 blocked and threatened AV in the activity room. It was discovered that W1 was exhibiting behaviors for several days prior to the incident, resulting in AV going into the closet in the bedroom and crying. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
