Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, on December 22, 2025, the AV was able to leave the facility thru a door that should have been locked. AV suffered a fall in the parking lot, which resulted in facial lacerations and a fracture to their neck. The facility failed to provide a safe environment, which is neglect of care and constitutes abuse.
