Findings: Alleged Victim (AV) lived at Respondent's facility. AV's service plan reflected that AV was at risk of elopement. However, the facility failed to supervise AV and to make sure that doors were locked and secured. On December 23, 2025, AV eloped the secured building, without staff knowledge, placing him/her at risk of harm. The facility failed to provide a safe environment to AV as ordered, which resulted in AV wandering out of the locked facility and placing AV at risk of harm due to being unsupervised in the community. which constitutes abuse by neglect as defined by OAR 411-020-0002(1)(b)(A)(ii)
