Findings: From November 2025 through the end of December 2025, Alleged Victim (AV) experienced persistent delusions and hallucinations, resulting in unpredictable and unsafe behaviors, including episodes of physical aggression toward others. During this time, the facility failed to protect AV from ongoing verbal mistreatment by other residents. Although the incident did not rise to the level of verbal abuse, the facility violated AV’s resident rights which is a violation of Oregon Administrative Rules.
